Problem in Gazette notification for name change in CDSL demat account

My school certificate name was G P Abarna. My pan card was given under the name Geetha Padmanabhan Abarna as pancard doesnt allow inclusion of initials. my initials are expansion of my mother and father name.I have opened a demat account 5 years back with my pan card, It was opened under the name Geetha Padmnabhan Abarna because as Pan card has it. My aadhar and voter id has the name Abarna without initials. recent SEBI circular mandates all the KYC documents should match aadhar to avoid freezing of demat account. so in order to avoid demat account closure I have changed my pancard name to abarna to match both aadhaar and pan name. Now the CDSL requires gazette notification to make changes in my demat account name. i have no photo identity proof to show that my name is GEETHA PADMANABHAN ABARNA except ITR filing acknowledgement for the last 7 years. Even the old pan card has been printed my name G P Abarna but in ITR database it was GEETHA PADMANABHAN ABARNA. i went to nearby notary to help me in gazette notification, but he says that he need any photo identity proof of GEETHA PADMANABHAN ABARNA. Can anybody help ? and advise me how to proceed further.
