Mother name is totally different in school certificates

Respected lawyers, My mother name is SABILA & due to the same name students my school teacher wrongly entered my mother name as NAFISHA which is totally different. We used 2 b 2 student with same name in class so my teacher recorded his mother name as mine & I couldn't know this cz in my time hand made results & certificates used to b given to us & on results & certificates they used to write father name only but in 10th class i got to know that but all teachers told me that it's too late nothing can b done now & i couldn't get my mother name corrected & After 12th class i started to write NAFISHA as my mother name in university cz my school documents show that name if i started to write real name which is SABILA then there would be problem i thought so i didn't do that. now I have to apply for passport. Kindly guide me What should i do now? & what name should i write in passport? I don't have any document showing NAFISHA except school documents. Plz help ne out in this! Thank u so much!
