• Name Change Affidavit from First Class Magistrate

My daughter name in her Xth Mark list and Aadhar card is different. When I asked the ICSE board, they advised to get the affidavit executed from the First Class Magistrate with the details.

We want to change the Mark sheet name with the Aadhar card name. 

Please let me know the procedure to do it. What is the format should be used for the same.

To update your daughter’s name on her ICSE mark sheet:

  1. Affidavit: Prepare an affidavit with a First-Class Magistrate, stating the name change from the mark sheet to the Aadhaar name. This can be done in Hyderabad (current address) or Mumbai (where she studied).

  2. Newspaper Notification: Publish a name-change notice in two local newspapers, one in English and one in the regional language, declaring the name change.

  3. Submit to ICSE: Provide the affidavit, newspaper clipping, and a copy of her Aadhaar to the ICSE board for the name update.

A Gazette notification may only be needed if ICSE requests it.

sane name would be reflected in school leaving certificate 

 

you have not mentioned details of your daughter name in birth certificate ,school leaving certificate and Aadhar card

I would suggest, you file it in Hyderabad itself being it the place of her current residence and school address. Newspaper notification orders will be carried out by the Hon'ble Court itself after it entertains your petition. Then you will have to get the same published in one local language newspaper and the other one in English national newspaper. No, it doesn't involve gazette notification.

You can prepare a sworn affidavit stating that both the names are one and the same and get it notarized and published in local newspapers. 

After that you may get it published in the government Gazette publication. 

You may keep all the relevant papers along with your certificates to prove and establish the other name too.

You cannot change the name in school certificate.

If you are settled at Hyderabad then you can process the name change there itself. 

Or if she's getting admitted in Hyderabad then also you can get it done at Hyderabad itself.

- You cannot change/rectify her name in the X certificate if it is issued by the board after one year of passing the said qualification. 

- Further, even this rectification is allowed if some mistakes has done by the board. 

- However, you can change her name in other documents after submitting an affidavit & news papers publishing. 

- You have to carry the activities in the same city , where this documents were issued.

Dear Client,

In the Present Scenario, your daughter's name in her 10th Mark list and Aadhar is different. When you asked about the same to the ICSE Board, they advised to get an affidavit executed from the First Class Magistrate with the relevant details. Herein, firstly, with respect to the format of the affidavit, the same must clearly state the discrepancy involved in your daughter's name on the mark list and Aadhar, and hence, your request to the ICSE Board to be instructed to correct the name, to match the government ID of Aadhar. Additionally, it should include the date of affidavit, signature of witnesses, and the place of signing. Next, for submitting the affidavit to the ICSE Board, a formal application for the same should be made, along with attaching the affidavit, Aadhar copy, Mark List and other relevant supporting documents. the Board would then verify the same, and initiate the process to verify the ambiguity present. However, a fee may be collected for the same, depending on the Board's policy.
