in froof what can i provide
You must have changed your name by gazette notification and publication in 2 local newspapers
if so only new name should be reflected in your certificate
Submit affidavit that you have changed your name by notification in official gazette and 2 local newspapers
- As per law, you cannot change/correct the name in matric certificate after 2 years from the date of issuing the certificate.
- However, you can correct the name in other documents after submitting the proofs
- Further, as the Polytechnic is based upon the matric examination , then they will adopt the matric name for admission , and even they will not accept any affidavit or declaration
- Hence , you should adopt the matric name and correct /change the name in all the documents after submitting the copy of matric .
- However, you can enquire from the Polytechnic that whether they accept the affidavit for name correction or not.
You prepare an affidavit declaring your new name alone, get it notarized and published in two local newspaper.
After that get it published in the government gazette publication, attach the copy of the gazette publication to the application and start using your new name everywhere after that.
You don't have to provide any proof other than the government gazette publication copy which will be the authority to recognise your new name
Dear client,
Changing your name can indeed create discrepancies across your educational and official documents. Given that your matriculation certificate contains both your old and new names while your other official documents only have your new name, it can pose challenges in maintaining consistency across your records. When it comes to your admission in the polytechnic and your request for only the new name to be on the certificate, you might want to explain your situation to the administration or the authorities in charge. You could present your updated documents and clarify that your legal name is now solely the new name. However, the decision ultimately rests with the institution's policies and procedures regarding name discrepancies on certificates. It might be beneficial to present legal documents showing the name change, such as a gazette notification or a legal affidavit, to support your request for the exclusive use of your new name on the certificate. Clearly explain your situation, emphasizing the importance of having only your new name on the certificate due to the discrepancy and the consistency you wish to maintain across your records. Ask if there's a process or department within the institution that handles name discrepancies or changes on certificates. They might have a protocol for handling such cases. If possible, consult with someone in authority at the institution beforehand to understand their policies regarding name changes on certificates.
When you're dealing with a name change across various documents, you'll want to provide legal documentation that supports this change. Here are some documents that could serve as proof of your name change:
1. Legal Affidavit or Deed Poll: A legal document stating your old and new names and affirming the name change.
2. Gazette Notification: In some countries, publishing a name change in the official government gazette serves as legal proof of the name change.
3. Court Order: If your name change was court-ordered, the court documents would serve as official proof.
4. Notarized Affidavit: An affidavit signed in the presence of a notary public affirming the name change.
5. Marriage Certificate (if applicable): If your name change was due to marriage, providing a marriage certificate could serve as proof.
6. Official Name Change Certificate: Some regions or countries issue specific certificates confirming legal name changes.
