change of name has to be by notification in official gazette and publication in 2 local newspapers in india 

 

2) also you need an affidavit of name change duly attested before indian consulate

You have to first create Name change Affidavit, Then Two give tow National Newspaper Advertisement one in English and other in local Language and third apply for Gazette State or Central. My suggestion is better to apply for Central Government Gazette.

Name will change by the order of the Collector of district you are domicile of. General application will submit and paper publication.

If you are still an Indian citizen then you may have to pursue the matter of name change process adopting due process of Indian law. 

You first have to prepare an affidavit declaring your new and old name,  give brief reason for changing your name,  get it notarized and then issue a public notice through newspaper publication in the local. 

After that you may have to get this name changed through a government Gazette publication of the state you belong to in India. 

You have to present in India to sign the affidavit and other related papers.

- You can publish a declaration /public notice in two leading news paper in US for the same. 

- Further, being an Indian , you can publish the same in official Gazette as well  after declaring that you have changed your name with new name, and the new name will be used for future purposes.
