• To add surname in birth certificate

I am from punjab but i was born in rajsthan hanumangarh district , and my birth certificate has been issued from rajsthan , to make passport i have changed my last name so now i want to change my last name in birth certificate as well but as i live in punjab but birth certificate has been issued from rajsthan the procedure is bit long and difficult so is there any other way to change last name in birth certificate ?

Make an application to the Registrar of Births and Deaths under your Municipal corporation

 

The letter should contain your old name’s details, your new name’s details, and a brief statement of the reasons for seeking a name change.

 

3)enclose affidavit of name change 

 

4)you should issue a small advertisement in the local newspaper, as a public declaration of nabe change

1. Make an affidavit for change of name, including your full name, reason for change, father's name, and residential address. Sign and notarize the affidavit.

2. Place an ad in a leading local daily newspaper with details of your old name, new name, address, name of witnessing/attesting authority, and date of the affidavit.

3. Submit the affidavit, original newspaper ad, proforma with your details and two competent witnesses' signatures, CD with scanned copy of application, two passport size photos, photocopy of valid ID proof, and a request letter with fee to the official gazette.

It will take 45 to 60 days for the gazette to publish the change of name.

- You can submit an application before the Tehsildar in Rajasthan from where this certificate was issued for changing /adding the surname , after submitting the passport copy. 

- You have to submit the other proof of name including an affidavit to confirm that you have change your name and further you will know with the new name.

You cannot change the name in your birth certificate as per your own desire, however this will not prevent you from changing your name, you can follow the name change procedure as per law, i.e., by preparing an affidavit, getting it publicized in newspaper and also in a government gazette publication etc. 

The copy of the gazette publication should be the proper document to change  the name in the passport .

You can't change you nane on birth m certificate. The copy of gazette notification will be enough for passport purpose
