Sir, My daughter Name Given Name : Arpita Bala Subramani and surname :Blank in passport , But actually the last name/Surname is Bala Subramani. Also How should i write in school records or should i write as First name Arpita and Middle name : Bala and Surname: Subramani. Pls suggest and tell me should i correct the passport also

- You can correct her name in the school record , if she has not passed the exam of school board till now , but the correction in the passport may time time and required many documents
- Hence, you can follow the name as mentioned in the passport , if it is not mandatory to be corrected.

To change your name in her passport, you have to apply for "Re-issue" of passport.

There is no surname at all in this because your name is Balasubramani hence 'Bala' cannot be your first name and 'subramani' your surname.
Therefore if your daughter name is Arpita Balasubramani, then there is no reason to add any surname in because there is no surname at all
If at all you would like your daughter to be known as 'ARPITA' alone and have her second name or middle name then you can apply for reissue of passport with the changed name.
In that case you can leave the surname column as blank.
It is not mandatory to get the surname recorded in the passport, even a single name without middle name or surname is accepted.
