Namaste! My mother needs to travel to the US tonight. Her passport has the name as “Nagaveni Anantha Subraya “ and DOB as [deleted] but her Covid certificate (based on Aadhar) has them as “CN Nagaveni and 05/05/1946”. Now, a vaccination cert is a must for her to board the international flight. Please advise what can be done to prove that she is the same person who is fully vaccinated. Thank you, Prasanna

1) Affidavit can be executed on Rs 100 stamp paper that both are IBR and same person
2) affidavit should be duly notarised
3) she must also carry some other identity documents which reflect name as in passport like election commission card ,driving licence etc
If immigration authorities refuse to accept the affidavit then she may face problems in boarding flight
she will have to change her name in aadhar card to reflect name in passport
The name in the passport seems to be totally different to that of the aadhar card, because the initials even if expanded may not suit the name appearing in the passport.
Since it is only covid certificate, you can show the aadhar card as evidence and explain the immigration department at the airport if they deny to allow her to travel.
If that is not possible then she may have to apply for reissue of passport with correct details by producing the documents to support her claim.

You can very well change her name as spelt in the passport on COWIN website. This is provided for there. You can mention her passport No. too in the Covid Vaccination Certificate.
You can get any of them changed as per which you want to retain
- She can execute an Affidavit on stamp paper after mentioning that her both names are same and of one person only .
- Further, she can correct her name in the said Covid certificate accordingly if required.
- Further, she can carry other documentary proof with her .
