The procedure to change the name and surname of a person is clearly mentioned by its website.

So follow the rules and do the same.

By making an affidavit and publishing the same in newspaper is largely prescribed but before that follow the method as prescribed by the Passport Authority in its website.

1. You have to apply to the Regional Passport Office (RPO) for change of DOB and surname by annexing all the documents which show that you were born on the DOB which you want to be incorporated in the passport and that you also have the surname which you want to be added on passport.

2. If the RPO does not accede to your demand then you may file a writ petition in the High Court against RPO.

apply to registrar of birth to rectify your date of birth in your birth certificate 

 

if they refuse you would need court orders to change date of birth in your passport

See Date of birth can be changed by producing Birth certificate before the concerned RPO seeking correction of same if there is no birth certificate then you shall be needing court orders to change same.

For change of surname you need to first make name change affidavit publish in 2 local newspaper and gazettee. 

After name is changed you file an application of name change before the RPO.

2)enclose school leaving certificate, aadhar card etc which reflect your year of birth as 1994

DOB can be change in Passport, easy procedure but here question is how you got year 1984 entered in your passport ? 

Do you have birth certificate ? what date mention in it.

And somebody told me when I go passport office for discussion my passport cancel I don't right or wrong -- Nothing like this.

Dear Sir,

An application for re-issue of passport in respect of change in personal particulars will have to be submitted for change in DOB.

But if you are holding current passport with validity and you are applying for changes in DOB than you will be fine a penalty of 5000.00 Rupees for it because you presented wrong showing DOB documents during previous passport issuance. If you have emergency than you can apply now but your application might stuck at RPO and they will cross-verify all DOB documents (Birth certificate/school leaving certificate) and send them to place of issue too. So it may take time up to 6 months approximately.

I advised you to wait till your passport expiry than you can apply for re-issue and also submitted for changes in DOB. If you follow this procedure than skip above steps and will get passport in normal time period.

While applying for an Indian passport the applicant should submit his date of birth proof such as birth certificate and correct name proof. On basis of the documents provided the passport is issued; so there will not be any question of change of date of birth in passport or difference in name. But while issuing a passport, the passport authority may make an error in date of birth on passport or in the name on ther passport.

If the error was from passport authority, then a new passport is issued with correct date of birth without any fee. If the error was from applicant then they charge a fee of Rs. 500 for issuing a new passport with corrected date of birth.

Any mistake in the passport application can be corrected during the appointment at the passport Seva Kendra provided you have the original documents and copies - copies can be made at the Passport Seva Kendra as well - to substantiate the change.

Also the details that you had in your last passport will not be changed unless you submit an affidavit and a newspaper advertisement along with your documents.

You can request a passport correction for an incorrect date of birth by submitting form DS-5504 to the address on the form. You can do this at any time during the validity of the passport with additional cost.

It will be necessary to submit your present passport and evidence of the correct date of birth.
