You can execute affidavit that sheel Kumari and Sushila are one and same person

2) affidavit should be on Rs 100 stamp paper

3) it should be duly notarised

The name in your educational certificates cannot be changed at this stage.

You may prepare an affidavit stating that your  mother's name appearing in 10th mark sheet to that of the same appearing in other educational certificates, though differing with each other, both are one and the same denoting the same person, you can get this affidavit notarised and issue a public notice in a leading newspaper and keep both the copies along with your certificates to  handle the situation that may arise in future.

You can correct the mistake in 10th marksheet with in 10 years of mark sheet issuing date.

If this limitation of 10 year is over then only option left with you is name declaration affidavit from your mother.

You can get an affidavit on a stamp paper mentioning that both the names i.e. sheela kumari and sushila belong to the same person. This will resolve your issue. Although you can get in touch with the relevant authority regarding the same issue.

1. OFFICIAL "change of name" procedures can be conducted ONLY thru "Govt. Gazette publication procedures, which takes about 30-90 days for official order and publications.

2. Once above is done, you would need to change "ALL" related documents (Aadhar, PAN, Passport, pending Agreements /Contracts /Bank Accounts .... ) "without any exceptions" except for Educational /Birth /other statutory Certificates .... which cannot be changed after 12 years).  You would need to give original /certified copy of the Govt. Gazette to all the aforementioned authorities for change in records & registers.

3. IF above is kept pending, THEN it would be a breach of trust /confidentiality /privacy on further transactions with the old name, after date of Govt. Gazette publication.

1. You have just simply given user rating as "TERRIBLE (1/5)".

2. Kindly provide reasons and explain as to how my advise is faulty or terrible.

Dear Sir,

As per your query i would like to tell you do not  worry  about it, it would not create any problem. During counselling you have to show your affidavit at the time of documents verification.

Procedure for name change in educational certificates

Affidavit

Seek the help of a local notary to make an affidavit for name change. You will have to explain the reason why you want to change your name. For instance, in certain communities, a woman is given a new name post marriage and she may wish to change it legally, or someone may wish to change it because either it’s too long or difficult to pronounce.

Publish in newspaper

Next, you need to publish the change of name in at least two local newspaper. It’s recommended that you publish one in an English daily and one in a regional publication. This is to announce your new name publicly. Remember to keep copies of newspaper with you.

Gazette notification

Once it’s published, you should then get it notified in the official gazette of the state you reside in. This can be done by filling the deed changing name form from the gazette office. Don’t forget to take duplicate copies. Submit the copies to the office for publishing in the Government press for gazette notification.

Verification

Now comes the part where your documents will be verified by the Department of Commerce and Transport secretary . For this, you will need to send

– A letter to the secretary stating that you want to change your name and request him/her to allow you to do that.
– The deed changing name form.
– At least 2 copies of local newspaper, which published your name.
– Attested copy of the affidavit.
– A copy of your present identity (PAN card, voter ID card, passport, etc).
